Otherwise, I'd be happy to set up a database myself, but this may require access to your control panel. I will need a host name, username and password, and optionally, a database name. I may need other information depending on specific circumstances, in which case I will ask. If you can't figure out any of this information, your hosting company should be able to tell you.
